Your First Dental Visit
Most new patients will enter the dental office through our hygiene department. Prior to your arrival, we’ll ask you for your email address over the phone so we can send you a request form. This should be filled out so we can request records from your previous dentist, if applicable. You can also complete new patient paperwork in our dental office or download and print it out to complete at home. Once we’ve received your records, captured X-rays, and performed a cleaning, one of our dentists will perform a detailed new patient exam.
New Patient Forms
As a new patient, we want to learn as much about you as possible. After all, it only allows us to better serve you moving forward. You can always complete your forms at the dental office if you’d prefer, but it’s also recommended to download them directly from our website and complete them at home! This can expedite your check-in process and save you time.
For many of our treatments, there are important steps that you’ll need to take in order to ensure a successful and smooth recovery period. Whether you just had a tooth extracted or a dental implant placed, we want to make sure you have everything you need to protect your smile and your oral health.
Dental Insurance Welcome
With dental insurance, getting the most important care for your family completed is easy and affordable. Our dental office staff will work with you directly to ensure that all of your benefits are maximized to the fullest extent. We are in-network with Humana PPO plans but we will file claims to most insurance providers.
