Humana PPO Coverage Availability & Fees

Humana offers three main PPO plans. Your specific coverage availability and fees will ultimately depend on your unique needs and preferences, but in general, here’s what you can expect:

  • Preventive Value plan. Touted as “budget-friendly coverage for preventive and basic care,” the Humana Preventive Value plan has a low monthly premium, unlimited annual maximum, a 1-time deductible, and 100% coverage for routine checkups and cleanings.
  • Loyalty Plus plan. For this unique PPO plan, your benefits will increase the longer you remain with Humana. The monthly premium can be considered middle-of-the-road, and includes some coverage for dentures and crowns. It also features a 1-time deductible.
  • Complete Dental plan. This option truly has everything (and might even include the kitchen sink). Of course, you’ll have to pay a higher monthly premium, but that certainly comes with value. You’ll receive 100% coverage for preventive care, 80% for basic treatments, and 50% for more major procedures.
